Conception prior to membership is ineligible for maternity sharing. Newborns must be enrolled within 30 days of birth. Household members (spouse, children under 26) eligible for Amaze even if not on plan. Monthly member fees and member responsibility costs for office visits, labs, and other tests do not count towards the Annual Max.
